A combination of mooring practices and weather conditions led to a mooring line being dragged overboard on vessels approach to port with a risk of entanglement in propulsion units.

During a routine marine inspection of two sister vessels it was observed that rope lines had been attached to the mooring winch control levers to facilitate ‘remote’ operation of the winches.
